People with a known allergy to azithromycin, other macrolide antibiotics (such as erythromycin or clarithromycin), or ketolide antibiotics should not take this medication, as it can cause severe allergic reactions, including anaphylaxis.

Individuals with a history of liver disease or jaundice related to previous azithromycin use should avoid the drug, as it can exacerbate liver problems. Azithromycin can also prolong the QT interval on an electrocardiogram, leading to potentially life-threatening heart rhythm disturbances. Therefore, it is contraindicated in patients with known QT prolongation, congenital long QT syndrome, or those taking other medications that prolong the QT interval.

Patients with severe renal impairment should use azithromycin with caution and under medical supervision. Additionally, it should be used cautiously in individuals with myasthenia gravis, as it may worsen muscle weakness. Always consult a healthcare provider before starting or stopping any medication to ensure safety and efficacy.
